چکیده

A 26-year-old woman was admitted with amaurosis fugax in the left eye and transient aphasia. Diffusionweighted MRI revealed a small infarction in the left insular cortex. Transoral carotid ultrasonography (TOCU)1 demonstrated left ICA dilatation with narrowing of the true lumen and intramural mass (figure 1A), which was consistent with hematoma on T1weighted MRI (figure 1B). Angiography, performed immediately after TOCU, showed luminal carotid stenosis as on TOCU (figure 2) and a diagnosis of ICA dissection was made. Two months later, the TOCU findings were improved (figure e-1 on the Neurology Web site at www.neurology.org). TOCU may be useful for noninvasive detection and follow-up of extracranial ICA dissection.2

Carotid and vertebral artery dissection syndromes.

Cervicocerebral arterial dissections (CAD) are an important cause of strokes in younger patients accounting for nearly 20% of strokes in patients under the age of 45 years. Extracranial internal carotid artery dissections comprise 70%-80% and extracranial vertebral dissections account for about 15% of all CAD. Aetiopathogenesis of CAD is incompletely understood, though trauma, respiratory infec...

Intracranial dissection and extracranial hypoplasia of the internal carotid artery.

issection of the intracranial portion of the internal carotid artery (ICA) is an uncommon cause of stroke or transient ischemic attack (TIA). Congenital anomalies of the ICA have been rarely associated with cerebral ischemia. However, unilateral ICA hypoplasia can be frequently associated with intracranial arterial anomalies and altered hemodynamics.1 We describe the case of a woman who had sud...

Teaching NeuroImages: internal carotid artery dissection presenting as Villaret syndrome.

A 63-year-old man presented with headache, hoarseness, and dysphagia. He had a left-sided Horner syndrome and wasting of the left sternocleidomastoid muscle. His tongue was deviated to the left on protrusion (figure 1). MRI and CT angiography revealed a distal left internal carotid artery dissection (figure 2). Villaret syndrome is a rare clinical entity comprising IX, X, XI, and XII cranial ne...

Sonographic findings in dissection of extracranial brain-supplying arteries.

Report entitled “Serial Transoral Carotid Ultrasonographic Findings in Extracranial Internal Carotid Artery Dissection” Yakushiji et al.1 In this article, the authors highlight the importance of early diagnosis in extracranial internal carotid artery dissection. They report a case of transient ischemic attack detected clearly with transoral carotid ultrasonography (TOCU) in the early stage of e...
